Patient has referral for home health care in the following cases:
- A patient has a chronic illness and physical disabilities, because of which the patient is unable to come to a medical institution;
- A patient is discharged from the hospital or from the Day stationary after surgical intervention;
- A patient diagnosed with stroke requires medical rehabilitation services and their provision started within six months of the onset of illness.

Health care services at home are provided by a certified nurse. The nurse carries out the injection of medication, the treatment of skin damage, the feeding of the patient through a probe, educates and trains the patient and his family members in caring activities.

In order to receive a state-funded service, it is necessary to have referral from a family doctor or referral from a medical institution after leaving a hospital or a day-care hospital due to surgical intervention.
